Assumption of Risk

I, the undersigned participant, understand that using exercise equipment, participating in fitness activities, and attending events at Lokahi House involve inherent risks, including but not limited to:

• Physical injury

• Muscle strains or sprains

• Equipment malfunction or misuse

• Slips, falls, or accidents

• Potential exposure to allergens or environmental hazards

I voluntarily choose to participate and assume all such risks, whether known or unknown.

Release of Liability

In consideration of being allowed to use the home gym and participate in activities or events at Lokahi House, I hereby release, waive, and discharge the property owner(s), event host(s), and any volunteers, guests, or affiliates associated with Lokahi House from any and all liability, claims, demands, or causes of action arising out of or related to injury, damage, or loss sustained while on the premises or participating in activities.

This release applies even if the injury is caused by ordinary negligence, except in cases of gross negligence or intentional misconduct as defined by applicable law.

Equipment Use

I agree to:

• Use all equipment responsibly and as instructed

• Avoid using equipment I am unfamiliar or uncomfortable with

• Immediately report unsafe conditions

• Not hold Lokahi House or its owner(s) liable for equipment failure or misuse

Medical Clearance

I certify that I am physically able to participate in exercise and event activities and that I have consulted a physician or voluntarily choose not to. I understand that it is my responsibility to monitor my own condition and stop immediately if I experience discomfort, pain, or dizziness.

Personal Property

I understand that Lokahi House is not responsible for lost, damaged, or stolen personal items brought onto the premises.

Event Participation

If attending special events at Lokahi House (including but not limited to group workouts, workshops, competitions, gatherings, demonstrations), I acknowledge that such events may involve increased physical activity and interaction with other participants. I assume all risks associated with these events and waive liability as stated above.
